NHS Long Term Plan
The NHS Long Term Plan (LTP) identified stroke as a clinical priority over the next ten years.
The LTP outlined how Stroke Services can work with partners to improve stroke care along the full pathway from symptom onset to ongoing care. This includes:
- Prevention
- Treatment
- Rehabilitation
The LTP highlighted the need for Integrated Stroke Delivery Networks (ISDNs) to bring people and organisations together to deliver the best possible care for their population.
West Midlands ISDN
The West Midlands ISDN was setup in 2021 and is hosted by University Hospitals Birmingham NHS Foundation Trust.
It consists of a project team including clinical leads for both acute and community services and we are governed by the Midlands Stroke Delivery Board to ensure we deliver our vision.
The team facilitates improvements in stroke services within the West Midlands by working in partnership with NHS providers, commissioners for stroke services and other stakeholders including the voluntary sector, academia and commercial organisations.
The ISDN supports the whole care pathway for stroke from prevention and pre-hospital care through to early-supported discharge (ESD), community stroke rehabilitation and life after stroke.
Our Vision
To support the development of high quality and equitable stroke services in the West Midlands, achieving the best clinical outcomes, patient experience and safety. We will achieve this by:
- being patient centred
- working collaboratively with stakeholders
- supporting our partners to work together towards service improvement
- encouraging the adoption of evidence-based practice in stroke services
